2. Prepare
-All intake forms will be emailed to you once appointment is confirmed.
-Please take time to complete these prior to your visit so it will be the best use of your time!
3. Telemedicine Visit
- your Email will contain link for Telemedicine visit. Just click when it's time to meet!
- Please have a recent blood pressure to report at each visit
-Visits can last 30+ min depending on your symptoms & goals.*
*national reported avg of 9-16 min spent in conversation with a health care provider/visit
Payment
-The initial intake packet contains credit card consent.
-Your credit card will be billed in full after each office visit.
(*Some insurances will reimburse for the visit. Each patient can contact their insurance for more information regarding reimbursement).
-Labs or imaging can be billed through your insurance
at the specific location of service.
-Initial Intake visit - $200
(compared to recommended fee of $330)
-Follow up visit -$175
(compared to recommended fee of $230)
Some patients opt to submit a claim to their insurance company for possible reimbursement. See below for more info.
